Programs: For over 50 years, we have provided high-quality, culturally competent health care to low-income residents. 80% of our patients are uninsured. We serve patients at three fixed-site clinics, which are complemented by mobile van sites located in communities of need. Our primary care services include integrated behavioral health, chronic disease management, and preventative screenings. We also offer certain in-house specialty care through volunteer clinicians, including heart and endocrine clinics in collaboration with NIH and Suburban Hospital. We regularly refer patients to other local resources to address social determinants of health such as food insecurity. Our case managers also help patients navigate to medical services beyond our program scope, including charitable specialty networks and affordable medications.
