Mission: To help individuals, families, and communities to break the cycles of substance abuse, family abuse, violence, crime and poverty.

Programs: Transitions is a multi-location social services organization providing substance abuse treatment, half-way house services, recovery programming, supportive housing and childcare. In 2014, transitions provided 125,618 days and nights of residential care, and 15,684 hours of outpatient counseling. Transitions provided services to 3,231 adults and children admitted to the various locations in 2014.
