Mission: Transition Projects provides individuals with the services, resources and tools they need to end their homelessness, secure housing, and maintain that housing.

Results:

Target demographics: Adults experiencing homelessness, with a special focus on veterans, people with disabilities, women, the LGBTQ+ community, and people of color

Direct beneficiaries per year: On any given day, we help meet the basic needs of more than 500 people through our Resource Center. On any given night, we provide a safe place to sleep for more than 800 people with nowhere else to turn. In any given year, we help place more than 1,000 people into affordable housing, and then support them in retaining that housing.

Geographic areas served: Portland-Metro area, Multnomah County, Clackamas County, Washington County

Programs: Transition Projects provides individuals with the services, resources and tools they need to end their homelessness, secure housing, and maintain that housing. We provide a wide variety of programs and services, ranging from basic needs (showers, clothing, ID services, and support groups) in our Resource Center to case management and employment services, short-term emergency shelter beds, and supportive housing. Whenever and wherever we can, we advocate for policies and initiatives that affirm the right of all people to a place they can call home.
